A. Access and Care Coordination
1. Pediatric Care Coordinator Model: Offer and implement a sustainable care coordination/patient navigation model in a variety of medical homes where Medicaid an CHP+ eligible children are treated in private practices in N. Aurora
a) AHA can help by identify funding sources, innovations group.

2. Aurora Behavioral Health Care Collaborative: Collaboration with Aurora Mental Health Center and Arapahoe House working to develop an interdisciplinary team of behavioral health treatment providers, including RNs and case managers. This collaboration is contemplating three separate initiatives: diversion from the ER to the ETU, continued support through case management, and/or research opportunities regarding cost savings and quality of life measurements.
a) AHA can help by identify which of the three initiatives are the priority in order to move forward with funding sources and implementation.

3. Hot Spotters Camden Model: Identify high utilizers of UCH and Children’s emergency departments and conduct community outreach to assure primary care follow-up and care coordination. These “frequent flyers” would be supported with on the ground with Nurse Practitioners and some form of community care coordinator

4. Faith-based Community Health Workers: Develop a network of churches in Aurora and identify and train informal “health ministers” within those churches who will provide health education, patient navigation, and advocacy. After attending a meeting with Gary Gunderson, Based on a model out of Memphis that seems like it would fit well with Aurora based on our current established relationship with MOP.
a) AHA can assist with researching parish nurses. Center for African American Health has extensive network and many historically black churches

5. Boomers Leading Change in Health CHWs: Patient navigators and community health workers, volunteers above 55, are trained by Boomers and housed in hospital/clinics in Aurora. Currently there are no BLCiH placed in Aurora. Boomers serving as AmeriCorps members and always looking for additional AmeriCorps sites.
a) AHA can help by identifying host sites (potentially pediatric offices)

6. Refugee-targeted Behavioral Health Promatoras: Promatoras providing health education and health care system navigation support to targeted refugee populations in Original Aurora.
a) AHA can help identify existing ESL and culturally competent health profession opportunities at the community college level.

7. University of Colorado Hot Spotters Group: Identify high ED utilizers and assure primary care follow-up and care coordination. The morning

8. Med Student Free Clinic: Open a free clinic staffed by CU Medical students to provide after hour services to indigent Aurora residents. Ideally open as elective on Fridays for law school students and med students starting in January 2013.

9. Colorado Alliance for Health Equity and Practice: Expand CAHEP clinic to MCPN South

B. Health and Wellness
1. BeWell Prevention Center: BeWell will open a new prevention center at Moorhead Recreation Center to provide health education and health and wellness resources. Driven by Block Captains, volunteers who have completed a 12 week program who then focus on screenings/prevention or health care policy.
a) AHA can help by getting to know “Frank” and identifying how the Block Captains can plug into the community. Grant money available for preventative opportunities at the Moorehead Rec Center

2. Grant from the Stapleton Foundation: AHA’s Health and Wellness Workgroup was awarded $9,000 ($3,000/year for 3 years) to design and implement health and wellness focused interventions.

1. Medicaid Adult without Dependent Children: Monitor rollout and implementation of AwDC expansion in Aurora. 1,700 out of the 10,000 available slots are dedicated to the Adams, Arapahoe, Douglas counties

D. Health Information Exchange
1. CORHIO: Continue to support and recruit Aurora providers into CORHIO pilot in Aurora. CO Access will be the first insurance provider that uses CORHIO. Due to AHA support, CORHIO will alter implementation

1. Pre Conception Peer Educator: National Office of Minority Health. Board developed will be fundraising in nature.
a) AHA can help by identifying pockets of youth who could potentially qualify for a stipend and expand this program. Currently targeting African American youth. Any suggestions for how to reach out to the Asian community, particularly Korean youth.

2. Aurora At Home: Group convened to discuss issues of homeless in Aurora. Not specifically focused on health, interested in broader issues related to homeless population.
III. Assignment: Pick one of the initiatives to join and reflect on the future of Aurora Health Access
A. How do we continue to harness this energy and not trip over each other?
B. Citywide focus! Although there are needs in N. Aurora, there are plenty in S. Aurora as well.
C. AHA needs to emphasize what it is accomplishing within the community. Community members need to know what the organization has done in order to be successful.

A. Delmar Park wants a community based plan for the park renovation. The first public meeting will be held on April 3 at the Peoria Elementary School at 6pm
